What happens at your D4 medical

Your appointment is a structured examination against the DVLA Group 2 medical standard, the same standard used for HGV (Class 1 and Class 2), LGV, PCV and bus drivers. It's considerably more thorough than a standard NHS check up, which is exactly why many NHS surgeries either don't offer it or have a waiting list of several weeks.

Here's what we cover with you:

• ID check. We confirm your photo ID, since the doctor signing your form is confirming they examined you personally.
• Medical history. Your past and current conditions, any surgery, and every medication you take. We don't have access to your NHS records, so bringing this information along makes the appointment run far more smoothly.
• Physical examination. Your heart, chest, nervous system, mobility and general fitness to drive a large vehicle safely.
• Blood pressure. Checked at rest and measured against the DVLA threshold (more on this below).
• Eyesight. Visual acuity and visual field tested against the Group 2 standard, with your glasses or contact lenses if you wear them.
• Your signed D4. Completed on the day, so you leave with the form finished. The doctor signing it is GMC registered and licensed to practise in the UK.

You don't need a referral, and you don't need to be registered with us beforehand.

What to bring with you

Bringing these along will help your appointment run quickly:

• Your D4 form, downloaded or collected in advance of your appointment
• Photo ID, either your passport or driving licence
• Glasses or contact lenses, plus a recent prescription if you have one
• A list of your current medications and doses
• Any relevant hospital letters, specialist reports or monitoring records, including cardiology investigations such as an echocardiogram or 24 hour tape, plus glucose readings or CPAP compliance data where they apply

Diabetes, sleep apnoea and other common conditions

A diagnosis of one of these conditions doesn't automatically fail you. What the DVLA looks for is evidence that a condition is controlled and monitored, not the diagnosis itself.

Diabetes is assessed by treatment type. If yours is managed by diet alone, metformin or GLP-1 medication, the detailed monitoring questions on the D4 don't apply. If you're treated with insulin, or with a sulphonylurea or Glinide such as Gliclazide, more information is required.

Obstructive sleep apnoea is common in this line of work, and very treatable. You must tell the DVLA if you have confirmed moderate or severe obstructive sleep apnoea with excessive sleepiness, or suspected or confirmed mild obstructive sleep apnoea causing excessive sleepiness for three months or more. You shouldn't drive until the sleepiness has resolved or your symptoms are controlled and you're following treatment properly, which for most drivers means using CPAP consistently. Once you're stable, bring your CPAP compliance data along and we'll talk through what the DVLA will expect from you going forward.
